According to the Louisiana State Police, the fatal incident occurred at approximately 5:27 p.m. at the intersection of Louisiana Highway 120 and Louisiana Highway 175.

Preliminary investigations indicate that a 2000 Chevrolet Van was traveling west on Louisiana Highway 120 when, for reasons still under review, the driver failed to stop at the intersection. The vehicle left the roadway and collided with the ditch embankment.
